Current Research Activities

Through the help of an award from the National Institutes of Health I have set up a clinical research unit in the emergency department at UCSF and now at Stanford University Medical Center. The unit is supported by research volunteers who are undergraduate students looking for an experience in a health care profession. The volunteers help screen and facilitate enrollment and data collection on patients involved in studies supported by the research unit. The unit also supports a clinical database which link the research databases with the clinical information systems within the hospital

The unit has supported three clinical studies since opening in July 2000. I have completed randomized trial comparing sutures versus conservative management of hand lacerations, which was selected for a plenary paper presentation at the 2001 Society for Academic Emergency Medicine (SAEM) annual meeting and accepted for publication in the British Medical Journal. I have also developed clinical guidelines for the need to admit patients with syncope. Over 600 patients were enrolled in the derivation set of this study. Our derivation set was recognized as a plenary session paper at the 2002 SAEM annual meeting and recently published in Annals of Emergency Medicine.

To date over 600 patients have been enrolled into a validation study for the syncope rule. By expanding to Stanford and hopefully other hospitals in the bay area we can focus on multi-center studies to implement the syncope rules and to expand my current RCT on the use of prophylactic antibiotics in dog bites. The findings from this trial will be combined with the economic model developed with colleagues in the Department of Health Research and Policy and the School of Business at Stanford to determine the most cost effective way to manage dog bites.
